Please arrive on time for your appointment.

ONLY ONE ADULT MAY BE WITH YOU.

Please shower the morning of your appointment. Do not tub bathe.

A State issued picture ID is required.

Minors (under 18) need a birth certificate, picture ID, and signed parental consent.

The parent must be present and provide a picture ID, and their name must be listed on the birth certificate.

A husband's consent is not required if you are married.

You must have an adult present (1 adult only over 18 years old) to drive you home. You may not drive yourself, walk, or take a bus home.

Absolutely no children allowed in the office at any time.

All fees must be paid in cash or credit card before services are rendered. Use of insurance arrangements must be made prior to your appointment.

Please leave all valuables at home. Wear no jewelry. We are not responsible for any lost, stolen, or damaged personal items or vehicles.

All tongue rings must be removed before arriving.

Do not eat, smoke, drink, or chew gum for at least 8 hours prior to your procedure.

Bring at least 1 maxi-pad with you.
